Prosthetic Dentistry of Mt. Lebanon, PC, is committed to making your first appointment as smooth and stress-free as possible. If you are able to print and complete your new patient forms before you arrive, you can help us minimize wait times and ensure that we have all of the information necessary to begin your first visit. If you are unable to print and complete your new patient forms in advance, please arrive 15 minutes earlier than your scheduled appointment time to complete them in our office.
